A 5th method is to import user specific information from an Excel file. This data will be part of the form as default values when the user opens the form from an email message that you sent using the group email feature. More information about this is available in the document

This form uses Ajax. This is a technique that is not supported by your browser. Unfortunately it is not possible for you to complete the form.
The url is the internet address (https://www...) of the form. In the top of your browser you will see this address. A part of this address is '?def_firstname=Alexander' that places John as a default value in this field.
