To register as a laser worker, you must fill out this form. If you will work in the laser lab for
more than 3 months,you will automatically receive an e-mail with instructions for the e-learning and test.
You have 3 weeks to completethe e-learning and pass the test.
After passing the test your local laser supervisor will instruct you about the local and general
laser procedures. You may have to sign for having understood the instruction and
knowing the rules and then will be granted access to the laser lab.
You will work under supervision of the local laser supervisor untill you are
experienced enough to get permission to work independently.
When you stay for less than 3 months, you should complete this form to register,
but do not have to do the e-learning and test. You will work under close supervision,
unless otherwise agreed.
